What is a Collateral Informant?

 A Collateral Informant is someone who knows the person being evaluated for ADHD.

For an adult, it might be a partner or friend from elementary or middle school.  Ideally the adult would ask a family member or friend who knew them before age 12 to complete a Wender Utah.


An adult being evaluated with no collateral informant? No family or friends who knew you before 12? See if you can find any report cards from childhood? You will be interviewed for examples you recall from childhood. Your report will reflect no collateral support, however.


If a child or teen, one collateral will be a parent, or parents.  The parent will complete a Vanderbilt, SNAP-IV or Conner's.


Another required collateral informant will be a teacher. This is because ADHD must cause impairment in both home and school. The Teacher will complete a Vanderbilt, SNAP IV or a Conner's.
